Medical data are not collected by default. This form is displayed only where required by a university, scholarship, visa, migration, insurance, accommodation or special-accommodation process.
I explicitly consent to the processing of health documents required for the specified procedure, which may include a general medical certificate, HIV test, hepatitis indicators, tuberculosis-free certificate, chest X-ray, vaccination records or disability and accommodation documents.
These data are used only to prepare the file and send it to the relevant university, insurer, governmental authority or accommodation provider. Internal access is restricted to system administrators and specifically authorised staff.
I understand that refusal to provide an officially required document may prevent the related procedure from being carried out, but does not justify collecting unnecessary additional data.
